The client reports experiencing fatigue and nausea following chemotherapy treatments and states, “It just makes me not want to eat. When I do eat, the food tastes funny.” Client teaching included encouraging the client to eat small meals several times daily, choose foods served cold or at room temperature, and avoid fried foods. The client was also advised to rest before meals to improve tolerance.
The oral mucosa was inflamed. The nurse instructed the client to avoid alcohol-based mouthwashes, increase daily water intake to 2 liters or more, and avoid hot foods to reduce mucosal irritation.
Laboratory findings were reviewed, and education focused on infection prevention due to chemotherapy-induced immunosuppression. The client was taught to avoid raw or undercooked meats, drink only pasteurized milk, boil water before drinking, and to take their temperature daily, avoid crowds, and perform frequent handwashing using antimicrobial soap.
The nurse also reviewed bleeding precautions. The client reported bleeding from oral ulcers. Teaching included using an electric razor, taking a stool softener if constipated, and avoiding coarse-textured foods that could further irritate the oral mucosa.
February
The client experienced a weight loss of 1.5 kg (3 lb) and a decrease in hematocrit but had improved WBC and platelet counts. Despite continued poor oral intake, the client reported increased water consumption and noted less inflammation of the oral mucosa. The client also reported having no recent bleeding episodes, indicating some improvement in mucosal integrity and clotting stability.
